Twofold Pricing Explained: Unlimited AI Medical Notes for $49 per Month (2026)

What Twofold costs and what's included: $49/month billed annually for unlimited AI clinical notes, coding, templates, and a signed BAA — with no per-note fees, no premium tier, and no contract.

Twofold pricing: a single flat-fee tile marked unlimited, feeding an unbroken stream of clinical notes that runs past the edge of the frame — one monthly price, no per-note metering.

Twofold costs $49 per month billed annually, or $69 per month billed month‑to‑month, for unlimited AI‑generated clinical notes. That single price includes every feature Twofold offers: unlimited notes, the full template library, CPT and ICD‑10 coding, patient progress tracking, telehealth and in‑person sessions, mobile and desktop apps, and a signed BAA on every plan — with no per‑note fees, no add‑ons, and no premium tier holding features back. Twofold offers a 7‑day free trial that requires no credit card.

This guide explains exactly what Twofold charges, what "unlimited" means in practice, what is and isn't included, how group pricing works, and how the annual cost compares to the alternatives. All figures were verified on 28 July 2026.

How Much Does Twofold Cost?

Twofold has one paid plan plus a group option. There is no higher tier to upgrade into, and no permanent free plan — evaluation happens through a 7‑day free trial.

Paying annually rather than monthly saves $240 per year — $588 instead of $828, a 29% reduction. Both options carry exactly the same features; the only difference is the billing cycle.

Current offer, July 2026
Twofold's month-to-month plan is $19 for your first month, then $69/month. That makes month one low-risk, but over a full year it comes to $778 — so if you expect to stay, the annual plan at $588 is still the cheaper route.

What "Unlimited" Actually Means

Unlimited means unlimited notes on the paid plan, with no monthly cap, no daily cap, and no metering of the things clinicians actually do. Specifically, Twofold does not charge per note, per session, per minute of audio, per regeneration, or per AI action.

  • Notes: no monthly or daily limit, whether you write 5 notes or 500.
  • Drafts, edits, and regenerations: unlimited, and not counted against anything.
  • Recording time: not metered — Twofold does not bill by the minute of audio.
  • AI assistant use: not sold as credits or capped as a separate allowance.
Why this matters
Metered pricing changes clinical behaviour. If a regeneration costs a credit, clinicians stop regenerating notes that need it. A flat fee removes that pressure from documentation entirely.

What's Included — and What Isn't Held Back

The more useful question about an AI scribe's pricing is usually not the headline number but what the headline number excludes. On Twofold's paid plan, the answer is nothing: there is one paid tier, and it contains the whole product.

  • Unlimited clinical notes across in-person, telehealth, and hybrid visits.
  • The full clinical template library — 70+ formats including SOAP, DAP, BIRP, and GIRP — plus custom templates you define.
  • CPT and ICD-10 coding suggestions, included rather than reserved for a higher tier.
  • Patient progress tracking across sessions, and reports.
  • The advanced AI assistant.
  • Mobile and desktop apps.
  • A signed BAA on every plan at no additional cost, and HIPAA-compliant handling throughout.
  • Works alongside any EHR — Twofold is EHR-agnostic, so there is no integration tier to buy into.

Twofold supports 50+ languages. The free plan also carries full feature access rather than a stripped‑down preview.

What is included in Twofold's single paid tier: unlimited notes, 70+ templates, custom templates, CPT and ICD-10 coding, progress tracking, reports, AI assistant, mobile and desktop, telehealth and in-person sessions, 50+ languages, signed BAA, and works with any EHR.

The 7-Day Free Trial

Twofold's free trial runs 7 days and requires no credit card, so it cannot roll into a charge you didn't intend. If you cancel during the trial you are never charged. The trial carries full access to the product — unlimited notes, every clinical template, coding, the AI assistant, and mobile and desktop access — rather than a stripped‑down preview.

Twofold does not offer a permanent free plan. If free‑forever is what you need, Heidi has one, and the comparison section below sets out what it does and doesn't cover.

You can start on the pricing section of our homepage without entering payment details.

Group and Practice Pricing

The Group plan starts at 3 clinicians and is priced on volume rather than published as a flat rate. It adds the things a practice needs beyond individual use: centralized admin billing, flexible seat management, shared team templates, standardized documentation, team onboarding support, and a dedicated success manager.

Typical group pricing lands around $49 per clinician per month — flat and unlimited — with volume discounts above that. Practices from 3 to several hundred clinicians are supported; book a call to get a quote for your size.

Cancellation, Contracts, and Commitments

There are no contracts and no commitments. You can cancel Twofold at any time directly from your account settings — there are no cancellation fees and no retention call to sit through. You can also switch between monthly and annual billing at any time.

How Twofold's Annual Cost Compares

The comparison that matters is annual cost at each vendor's cheapest plan that removes the note cap, since that's what daily documentation actually requires. All figures below come from each vendor's own pricing page, verified 28 July 2026:

Heidi Practice (billed annually)

On unlimited notes alone, Twofold is $360 a year less than Freed Core and $732 less than Heidi Clinician. Coding changes the picture further: both Freed and Heidi reserve CPT and ICD‑10 coding for their top tier, so a clinician who codes is really comparing Twofold at $588 against Freed Premier at $1,248 and Heidi Practice at $2,160 — $660 and $1,572 a year less respectively.

Three details worth knowing. Heidi's listed prices are per user and exclude applicable sales tax, so the invoice runs higher than the sticker; Twofold's $49 includes tax. Heidi's Clinician plan is $110 per user per month only when billed yearly — billed monthly it is $150, or $1,800 a year. And Freed currently displays Starter at $39 against a struck‑through $59, so that plan's price advantage may be promotional.

Two honest caveats, because a pricing comparison that only points one way isn't worth much. First: at Freed's current $39 rate, a clinician writing fewer than 40 notes a month pays $468 a year against Twofold's $588 — genuinely cheaper, though at the $59 list price it would be $708 and the advantage reverses. Second, and more significant: Heidi offers a permanent free tier and Twofold does not. Heidi's free plan covers unlimited transcription, standard templates, and task management, with Ask Heidi, advanced templates, patient and session linking, and personalizations all limited, and no coding. If that covers your work it is free indefinitely, where Twofold gives you 7 days. Heidi's paid trial is also 14 days to Twofold's 7.

Our Freed pricing guide and Heidi pricing guide go deeper on each vendor's tiers and what sits behind them.

Annual cost for unlimited AI clinical notes, verified July 2026: Twofold $588 per year with coding included, Freed Core $948 without coding, Freed Premier $1,248 with coding, Heidi Clinician $1,320 without coding, Heidi Practice $2,160 with coding. Freed Starter is $468 but capped at 480 notes per year. Heidi offers a permanent free tier with several capabilities limited; Twofold offers a 7-day trial only.

Bottom Line

Twofold costs $49 per month billed annually — $588 a year — for unlimited clinical notes with every feature included: coding, templates, progress tracking, the AI assistant, all devices, and a signed BAA. There is no higher tier, no per‑note fee, and no contract. Start with the 7-day free trial — no credit card required — and see what your own notes cost before you pay anything.

FAQ

Frequently asked questions

  • How much does Twofold cost?

Twofold costs $49 per month when billed annually ($588 per year) or $69 per month billed month‑to‑month ($828 per year). As of July 2026 an introductory offer makes the first month $19 on the monthly plan, after which it is $69 — so a full year on monthly comes to $778, still more than the $588 annual plan. Both plans include unlimited clinical notes and the full product — the template library, CPT and ICD‑10 coding, patient progress tracking, the AI assistant, mobile and desktop apps, and a signed BAA on every plan. Twofold offers a 7‑day free trial that requires no credit card, but no permanent free plan. Group pricing for practices of 3 or more clinicians is quoted on volume. Verified 28 July 2026.

  • Does Twofold charge per note or per minute?

No. Twofold charges a flat monthly fee and does not bill per note, per session, per minute of audio, per regeneration, or per AI action. Unlimited means there is no monthly or daily note cap — the price is the same whether you write 5 notes or 500 in a month. There are no credits to buy, no overage charges, and no add‑ons.

  • What is included in Twofold's $49 plan?

Everything Twofold offers. There is a single paid tier, so no features are held back for a higher plan: unlimited notes, 70+ clinical templates (SOAP, DAP, BIRP, GIRP and more) plus custom templates, CPT and ICD‑10 coding suggestions, patient progress tracking, reports, the advanced AI assistant, telehealth and in‑person session support, mobile and desktop apps, 50+ languages, and a signed BAA at no extra cost. Twofold is EHR‑agnostic, so it works alongside any EHR without an integration tier.

  • Does Twofold have a free trial, and do I need a credit card?

Yes — Twofold's free trial runs 7 days and requires no credit card, so it cannot convert into a charge you didn't intend. If you cancel during the trial you are never charged. The trial gives full access to the product rather than a limited preview: unlimited notes, every clinical template, coding, the AI assistant, and mobile and desktop access. Twofold does not offer a permanent free plan — for a free‑forever option, Heidi has one, though its free tier limits advanced templates, patient linking, personalizations, and Ask Heidi, and excludes coding.

  • Can I cancel Twofold at any time?

Yes. There are no contracts or commitments. You can cancel directly from your account settings at any time, with no cancellation fee and no call to make. You can also switch between monthly and annual billing whenever you like. If you cancel during the 7‑day free trial, you are never charged.
