Guides and templates for writing clear case notes, choosing an AI scribe, and documenting risk.
Twofold is an AI scribe, also called an ambient scribe or AI note taker, built for Australian mental health clinicians: psychologists, counsellors, psychotherapists, mental health social workers and psychiatrists. It listens to a session, or to a summary you dictate, and drafts a structured case note in your own style in minutes.
What Australian clinicians look for, and what Twofold offers:
Yes. Twofold is an APP entity under the Privacy Act 1988, so the Australian Privacy Principles apply to us directly, not only through a contract with your practice.
Ahpra's guidance, Meeting your professional obligations when using Artificial Intelligence in healthcare (August 2024), applies to all registered health practitioners, including psychologists. In short:
Twofold fits these expectations: every note is a draft you review and approve, recordings are not kept, and client data never trains AI models.
In the United States. Twofold runs on Microsoft Azure and Google Cloud data centres in the US and does not offer Australian hosting. Our privacy policy discloses this overseas disclosure, as APP 8 requires.
No. Audio is processed to produce the transcript and note, then deleted. No recording is written to disk at any point. Your finished notes stay in your account until you delete them.
No. Twofold never uses your audio, transcripts or notes to train AI models, ours or anyone else's. Your clients' information is used only to produce your documentation.
Ask before the session, explain it in plain language, and note the client's answer in their file. Ahpra, PACFA and the ACPA all expect informed consent, and recording a conversation without consent can breach state surveillance devices laws.
A script you can adapt:
"With your permission, I use a secure AI scribe called Twofold to help me write my notes. It listens to our session and drafts a note that I then check and edit. The recording is deleted as soon as the note is written, and your information is never used to train AI. You can say no, or change your mind at any time, and it won't affect your care. Are you comfortable with me using it today?"
PACFA's Good Practice Guideline on integrating artificial intelligence in counselling and psychotherapy (2024) asks counsellors to protect confidentiality, get informed consent, keep data secure, and be trained to use AI tools competently. The confidentiality and record-keeping duties in your professional body's code, including the ACA's, apply to notes drafted with AI just as they do to notes you write yourself.
In practice: choose a tool that doesn't keep recordings or train on client data, tell clients you use it, and check every note before it goes into the file.
Yes. Twofold drafts, you decide. Every note is a draft you read, edit and approve before it goes into your client's file, which is what Ahpra, PACFA and the ACPA expect. Twofold learns from your edits, so drafts need less correcting over time.
Yes. Copy the finished note from Twofold and paste it into Cliniko, Halaxy, Power Diary, Coreplus or any practice software with a notes field. Twofold needs no integration or access to your practice software, and you can shape your templates to match the sections your software uses.
SOAP, DAP, BIRP, GIRP, case notes, intake and assessment notes, treatment plans, discharge summaries and client letters. You can also build your own templates, for example a progress letter to a referring GP.
Twofold is free for 7 days, with no credit card. After that, your first month is about A$27 (US$19), then about A$98 a month (US$69), or about A$70 a month billed annually (US$49). You're billed in US dollars; the Australian dollar amounts use the exchange rate on 14 October 2026, so your bank's conversion may differ slightly.
Yes. Use Twofold for telehealth or face-to-face sessions, on your computer or your phone. If you'd rather not record, dictate or type a short summary after the session and Twofold writes the note from that.
Usually a minute or two after the session ends. Most clinicians then spend a few minutes reviewing the draft, instead of writing the note from scratch after hours.
